Eligibility and Deadlines
Special Enrollment Period
If you experience a life event such as marriage, divorce, birth of a child, or change in a spouse's benefits, you may be able to make a change to your plan outside of open enrollment. Contact Innovative at (540) 613-8820 for assistance within 30 days of event.
Benefit Plan Notices
Your annual required Compliance Documents are posted in your online portal for review. These will include:
Compliance Notices: Details on various mandated benefits, outlines regarding interaction with other plans, and other rights related to benefits and access.
Summary Plan Description Wrap (SPD Wrap): Details on eligibility, access, and plan operations are not outlined in the benefit summaries.
Premium Conversion Plan (section 125 PCP): Details on eligibility, access and plan operations of the section 125 salary reduction program for those eligible. This permits you to pay your portion of premium pre-tax.
Summary of Benefits & Coverage (SBC): Standard format benefit comparison outline mandated by the Affordable Care Act.
Key Terms
Coinsurance: The percentage of health care costs you pay after meeting your deductible.
Copay: A fixed amount you pay for eligible services or prescriptions (you may not have to pay your deductible first).
Out-of-pocket-maximum: The most you will pay for health care in a year. Once you reach the maximum, you pay $0 for eligible healthcare expenses.
Deductible: The amount you pay before your plan begins to pay for eligible health care expenses.
Preventive care: Health care services that prevent illnesses or diseases, such as annual check-ups, screenings, and immunizations.
